Executive Health Options (EHO) is an international major medical insurance plan that covers hospital and medical expenses, and more. You choose the location, physician and specific hospital that is right for you, in Canada, the U.S. or the world.

All benefits are subject to the provisions, limitations and exclusions contained in the Certificate of Insurance. Coverage area excludes countries restricted by U.S. economic sanctions and embargo programs and/or other applicable trade sanction laws.

How is EHO different?

Executive Health Options (EHO) pays up to $5 million U.S. lifetime in funding for major medical health coverage. You and your doctor choose the doctor and hospital which are best for you. Our referral and information services are world renown. EHO has a wide range of deductibles and is renewable yearly.

What Medical Benefits are included?

In-Patient and Out-Patient hospital costs and many Out-of-Hospital costs. Benefits are subject to deductibles and limits

What Services are Included?

Pre-claim:
Health Information Libraries: Authoritative reports organized for easy access and maximum use by our members.

Services to access international care:
International claims specialists, medical professionals and client relations specialists are available 24 hours a day, 7 days a week while your EHO Plan is in effect, to answer your questions and respond to your needs. HCC Medical Insurance Services, LLC is the Plan Administrator.

Other Support Services:
A wide range of Customer Services including referrals, travel assistance and emergency assistance are available 24 / 7.

Will the Plan cover me in Canada?

The Plan covers eligible medical expenses in Canada which are not covered by any government or group plan you may have (after the deductible and coinsurance).

Can anyone apply for EHO?

Canadians—and non-Canadians—, from age 14 days to 74 years are eligible to apply. Those persons who have and maintain a healthy lifestyle are most likely to be accepted without riders or increased premium.

Does EHO cover...Citizens of other countries?

YES. (Except for U.S. citizens — including dual U.S./Canadian citizens — who are permanently residing in the U.S.)

Does EHO cover...Canadians living in the U.S.?

YES, if a Canadian—citizen or landed—is currently in Canada and will complete the application in Canada, with a Canadian address and phone number at the time of underwriting. If a non-U.S. citizen is currently residing in the U.S., an Affadavit of Unavailability of Insurance must be completed to satisfy U.S. regulations.

Does EHO cover... Children?

YES, the first two children aged 14 days to 9 years are free when both parents are insured under the plan. Children with one parent or guardian insured under the plan are charged the Dependent Child rate (for ages 14 days to 9 years and 10 to 18 years). Dependent Children with their own policies pay the age 19 to 24 male rate.

Does EHO cover...Seniors?

YES. Eligibility for the standard Executive Health Options plan ends at age 75, but for those members who joined before age 65 and have maintained coverage for at least 10 years, the Executive Health Senior Plan is available with no medical questions. The Senior Plan offers many of the same benefits and limits as the standard plan. The Senior Plan deductibles available are $5,000, $7,500 and $10,000. The Overall Maximum Limit is $1 million and the Human Organ Transplant and Emergency Medical Evacuation benefits are deleted.

How is my application evaluated?

Medical Underwriting considers a wide range of information to predict the risks and expected costs for each of our applicants. Underwriters will evaluate factors such as your age, health history, lifestyle and family health history. Our premiums are for standard risks. They are based on healthy people. If the underwriters determine an increased risk in your case, they may assess additional premium or assign riders to the policy, or they could decline coverage. You will be asked to approve all changes before the policy is issued.

Is EHO renewable?

YES. EHO is annually renewable to age 75. There are no medical questions at renewal.

What happens at Renewal?

When you renew your coverage the cost is likely to increase. Your premium is priced according to the claims experience of an entire class of insured persons (those of your age and sex and level of risk insured in the same year you were) plus economic and medical inflation factors. Your EHO advantage is that this process is based on years of experience wordwide.

Will my Claim affect Renewal

NO. Renewal is only subject to your continued eligibility and participation in the Plan.

What is the coinsurance?

Coinsurance applies to covered treatment received in the U.S. and Canada after the deductible is applied. Within the U.S. Preferred Provider Network (PPO), 100% of eligible medical expenses paid directly by HCCMIS are covered. If you decide to use a doctor or hospital outside of the PPO Network, 80% of the first $5,000 after deductible is covered (the 20% being coinsurance); then 100% is covered. Outside the U.S. and Canada, after the deductible,100% of eligible medical expenses are covered.

Why do I have to pay in U.S. dollars?

All benefits, premiums and compensation are in U.S. $

Does the plan cover...Second opinions? diagnostic medical testing?

When diagnostics are required, the Plan will cover the expenses incurred for physician and related professionals or the use of technology or facilities, including tests such as an MRI or a CT scan. This testing must be integral to the treating physician’s attempt to properly and correctly diagnose a condition.

Does the plan cover...Pre-existing conditions?

Pre-existing conditions are covered the same as any other accident or illness,

  • as long as the conditions are fully disclosed on the application
  • and not limited or excluded by rider or other provision of the policy.

Pre-existing conditions include any injury, illness or mental health disorder that existed prior to your initial effective date, including chronic or recurring conditions.

Does the plan cover...Transplants?

Covered transplants include Heart, Heart/Lung, Kidney, Kidney/Pancreas, Liver and Allogenic and Autologous Bone Marrow. They are covered the same as any other illness. In addition, some of the Donor costs are covered.

Does the plan cover...Physical exams?

For age 30 or older: $250 per member per cerfificate period; female members age 40 or older or women at risk: $100 per certificate period for a screening mammogram. Benefits are available after 12 months of continuous coverage. This benefit is not subject to the deductible.

Does the plan cover...Drugs?

Prescription drugs prescribed by a physician for treatment of a covered injury or illness are covered, in hospital and out of hospital. This benefit is subject to deductible and coinsurance.

Does the plan cover...Sports injuries?

Covered activities include amateur sports or athletic activities which are non-contact and engaged in by the Member solely for leisure, recreational, entertainment or fitness purposes (unless such sports or activities are otherwise excluded).

Organized team contact sports are excluded. A Sports Rider is available at additional cost for contact amateur sports such as hockey and soccer, heli-skiing , SCUBA diving (with restrictions).

Does the plan cover...Dental costs?

Only treatment of natural teeth injured in a covered accident are covered. A Dental Rider is available at additional cost.

Does the plan cover...Life & accidental death?

An Optional Term Life and AD&D Rider is available at additional cost.
